ADS/OE Hab Assistant Float
Vocational Services Provides support services, training, delegated nursing, and personal care for adults with multiple disabilities (i.e., physical, developmental and/or mental) in basic daily living, recreational and work-related habilitation activities to maximize their independence and community integration in accordance with their Individual Plan (IP) goals. Carries out IP programs and works cooperatively with other staff members in providing comprehensive services to program participants to ensure that their needs are met efficiently and that they are served in a safe, clean and comfortable environment. The ADS/OE Habilitation Assistant Float is cross-trained to perform the job duties and established routines of the ADS Assistant in the Adult Day Support program at all program locations (Wolstein & Westlake), as well as the Habilitation Assistant in the Organizational Employment program (production area). The Float might also provide basic client support coverage at a Community Employment work site. The Float will work in the program and location where staff coverage is most needed at a given time. We require a high school diploma or GED, one year’s direct care experience in an ADS activity room, sheltered workshop, or residential setting, with additional training, coursework and/or relevant experience serving adults with multiple and/or severe disabilities required; two year’s experience preferred, physical capacity to perform lifting (minimum of 50 lb.) and assume body positions related to routines of personal care of clients (e.g. pushing, pulling, bending, twisting, crouching, reaching, stooping, and kneeling) required, ability to complete state-approved medications administration course and competency to perform delegated nursing tasks as evaluated by the agency nurse required, verbal and writing skills necessary to effectively communicate and document client needs, a professional manner and willingness to work as part of a team, a valid Ohio Driver’s License with no more than two moving violations and reliable daily transportation.
In Home Care Assistant
1st Shift, 2nd Shift East & West Sides No certification is required. These positions are located in either one of our group homes or in the clients’ own homes. These individuals provide direct care such as meal preparation, feeding, bathing, dressing, toileting and housekeeping to adults with disabilities . We require a high school diploma or GED, a valid driver’s license with no more than 2 moving violations, reliable transportation, must be at least 21 years old and have the ability to lift 50 pounds or more.
Occupational Therapist (OTR/L)
Children's Services With general supervision, plans, organizes and participates in medically oriented occupational therapy programs to habilitate children with developmental disabilities in center-based or school based programs. We require a combination of education and experience normally represented by a Bachelor’s Degree in Occupational Therapy, licensed by the State of Ohio, and certification by the American Occupational Therapy Association and the ability to lift a minimum of fifty (50) pounds.
Physical Therapist
Children's Services We are looking for individuals to provide physical therapy to children with disabilities. You will work with children in school based settings and in our pediatric clinic. Work is done in collaboration with school, community and other professionals to provide coordinated family-oriented, child-centered services to our clients.
With administrative supervision, diagnoses neuro-muscular disorders and undertakes corrective measures to improve the client’s functional ability. Participates in the development of a client-centered, family-oriented treatment plan for the client. Provides evaluation and treatment services in both independent and multi-disciplinary modalities. Teaches family members how to carry out physical therapy exercises/treatment at home. Works in collaboration with school, community and/or other related professionals as needed to provide coordinated family-oriented, child-centered services. Provides Physical Therapy to school districts as assigned.
QUALIFICATIONS: A degree in Physical Therapy from an approved school. Licensure in the State of Ohio. Experience in working with children in team, collaborative settings. Ability to lift at least 50 pounds.
