RESIDENTIAL SERVICES - INDEPENDENT LIVING PROGRAM

The Independent Living Program serves adults with developmental or acquired disabilities who are able to financially maintain a private residence. The program helps individuals locate accessible housing in neighborhoods with easy access to banks, grocery stores, places of worship, public transportation and other frequently used services. Daily support and homemaking services are provided in the individual's residence, and individuals receive assistance in developing social supports and in taking full advantage of the opportunities and services available within their communities.

Daily Support Services include assistance in decision-making, accessing community resources and services, financial management, healthcare coordination, homemaking, meal planning and preparation, personal care, planning and participation in social activities, coordinating transportation, and training in daily living skills.

Individuals served through the Independent Living Program must have a developmental or acquired disability and may require minimal to around-the-clock support services. Eligibility requirements include the ability to participate in the personal decision-making process, possession of basic self-help skills or the ability to communicate one's needs, and the ability to financially maintain a private residence with personal or entitled funds.
